Medical Forms

All participants on courses at Outward Bound are required to complete an Outward Bound medical form with a doctor which is due eight weeks before your course. The medical is to ensure that conditions that could prevent safe completion of the course are identified. The medical is valid for 90 days and must be valid for until the last day of your course. If your form expires before the end of the course you will be asked to get your Doctor to re-sign your form. The medical is completed at your expense and usually costs in the region of $30-70.

The medical form will be sent out with your confirmation pack and is also available below. There are additional sections for people who have asthma or who have had a mental health condition. Final confirmation onto the course is on receipt and checking of your completed medical, and is at the discretion of the Outward Bound Trust of New Zealand.

If you are not a New Zealand citizen then you will need to complete an International Medical Form.
